By accessing or using the AmyloidTest website (amyloid-test.com) and ordering our at-home sample collection kit or laboratory analysis service, you agree to be bound by these Terms of Service ("Terms"). If you do not agree, do not use our service. These Terms constitute a legally binding agreement between you ("Customer," "you," or "your") and AmyloidTest ("we," "us," or "our").

AmyloidTest provides an independent laboratory microscopy service. Our service includes: - An at-home dried blood sample collection kit shipped to your address - Prepaid return shipping to our partner laboratory in Mexico City, Mexico - Thioflavin T (ThT) fluorescence microscopy analysis of your dried blood sample - Physician-supervised, AI-assisted analysis of microscopy findings - A Photographic Microscopy Report delivered to your secure portal or email **What Our Service Is:** - An informational laboratory data service - A photographic microscopy report based on your submitted sample - An observational screening tool aligned with published post-viral coagulation research **What Our Service Is NOT:** - A medical diagnosis - A clinical diagnostic test or medical device - A substitute for professional medical evaluation, advice, or treatment - An Alzheimer's-specific assay (we do not measure Beta-amyloid 42/40 peptide ratios or perform PET imaging) - FDA-approved, COFEPRIS-approved, or approved by any other national medical regulatory authority Our service is provided for educational, informational, wellness, and research purposes.

**Pricing:** - USD: $160.00 (United States and international) - MXN: $3,200.00 (Mexico) Prices include the collection kit, prepaid return shipping, laboratory processing, and delivery of your Photographic Microscopy Report. **Payment Methods:** - International/USD: Processed via Stripe (stripe.com) - Mexico/MXN: Processed via Mercado Pago (mercadopago.com) By submitting payment, you authorize our payment processor to charge the stated amount. All transactions are processed in the currency selected at checkout. Your payment information is handled directly by our payment processors and is not stored on our servers. **Order Confirmation:** After placing your order, you will receive an email confirmation with your order details and estimated shipping timeline. **Price Changes:** We reserve the right to modify pricing at any time. Changes will apply to future orders only and will not affect orders already placed and paid for.

**Your Responsibilities:** - Follow the collection instructions included with your kit precisely - Collect the sample using the provided safety lancet and glass slides - Allow the blood spot to air dry completely before packaging - Return the sample using the prepaid mailer within 30 days of collection - Do not use coverslips or add any substances to the sample **Sample Requirements:** - The dried blood sample is classified as an Exempt Human Specimen for shipping purposes - No dry ice, special couriers, or cold chain is required - Room temperature shipping is acceptable **Sample Quality:** We and our laboratory partner will make reasonable efforts to analyze your sample. However, if the sample is: - Insufficient in quantity - Contaminated or improperly collected - Damaged during transit - Received more than 60 days after collection We reserve the right to request a second sample at no additional charge, or to issue a refund minus the cost of materials and shipping. **Biological Material:** Your dried blood sample becomes the property of our laboratory partner upon receipt for the purpose of analysis. After analysis and any applicable retention period, samples are disposed of in accordance with applicable biological waste regulations.

**What You Receive:** A Photographic Microscopy Report containing: - High-resolution microscopy images of your sample under Thioflavin T fluorescence - AI-assisted pattern analysis observations - Physician review notes and commentary - A summary of detectable findings **Turnaround Time:** Results are typically available within one (1) business day from when the laboratory receives your sample. Total time from kit delivery to report delivery is typically 5–7 business days including shipping. These timelines are estimates and not guaranteed. **Interpretation:** Your report contains observational laboratory data. The findings should be reviewed by a qualified healthcare provider for clinical interpretation. We do not provide medical advice, diagnosis, or treatment recommendations. **Accuracy:** While we employ physician-supervised, AI-assisted analysis using established fluorescence microscopy methodology, no laboratory service can guarantee 100% accuracy. Results are dependent on sample quality, collection technique, and the inherent limitations of observational microscopy. **Your Data:** Your report is your data. You may share it with any healthcare provider of your choosing. We will not share your individual results with third parties without your explicit consent, except as required by law.

All content on amyloid-test.com, including but not limited to text, graphics, logos, images, software, and the design, selection, and arrangement thereof, is the property of AmyloidTest or its licensors and is protected by intellectual property laws. Your Photographic Microscopy Report is provided for your personal use. You may share it with healthcare providers but may not reproduce, distribute, or commercially exploit report content without our written permission. Our AI models, microscopy methodologies, and analysis algorithms are proprietary and constitute trade secrets. You may not reverse-engineer, decompile, or attempt to extract or replicate these systems.

By using our service, you agree not to: - Use the service for any unlawful purpose - Submit samples that are not your own without proper authorization - Misrepresent your identity when ordering - Attempt to interfere with our website, systems, or laboratory operations - Redistribute or resell our kits or services without authorization - Use our trademarks, branding, or content without permission - Submit falsified, contaminated, or hazardous materials - Use reports or results in a misleading or deceptive manner We reserve the right to refuse service to anyone for any lawful reason.

**IMPORTANT — PLEASE READ CAREFULLY:** TO THE MAXIMUM EXTENT PERMITTED BY APPLICABLE LAW: (a) Our service is provided "AS IS" AND "AS AVAILABLE" without warranties of any kind, whether express, implied, or statutory, including but not limited to implied warranties of merchantability, fitness for a particular purpose, and non-infringement. (b) AmyloidTest, its officers, directors, employees, agents, laboratory partners, and affiliates shall NOT be liable for any indirect, incidental, special, consequential, or punitive damages, including but not limited to loss of profits, data, use, goodwill, or other intangible losses, resulting from: - Your use of or inability to use the service - Any results, findings, or omissions in your Photographic Microscopy Report - Any action or inaction taken by you or your healthcare providers based on your report - Delays, errors, or failures in sample shipping, processing, or analysis - Any unauthorized access to or alteration of your data (c) Our total cumulative liability to you for any and all claims arising out of or related to the service shall not exceed the amount you paid to us for the specific service giving rise to the claim. (d) This service does not constitute a doctor-patient relationship. We are a laboratory data service, not a healthcare provider. You are solely responsible for seeking appropriate medical advice and treatment from qualified healthcare professionals.

You agree to indemnify, defend, and hold harmless AmyloidTest, its officers, directors, employees, agents, laboratory partners, and affiliates from and against any and all claims, damages, losses, and expenses (including reasonable attorneys' fees) arising out of or related to: - Your use of our service - Your violation of these Terms - Your violation of any applicable law or regulation - Any action taken by you or your healthcare providers based on information contained in your report

**Before Shipment:** You may cancel your order at any time before your kit has been shipped for a full refund. Contact support@amyloid-test.com with your order number. **After Shipment, Before Sample Return:** If your kit has shipped but you have not yet returned your sample, you may request a refund minus the cost of materials and shipping ($40 USD / 800 MXN). **After Sample Return:** Once your sample has been received by the laboratory, the service has been initiated and is non-refundable. We will make every effort to provide your report; if we are unable to produce a report due to sample quality issues, we will offer a complimentary replacement kit. **Refund Processing:** Approved refunds will be processed to your original payment method within 10–15 business days.

**Governing Law:** These Terms are governed by the laws of Mexico, without regard to conflict of law principles, as our primary operations and laboratory are based in Mexico City. **Dispute Resolution:** Any dispute arising from these Terms or your use of our service shall first be attempted to be resolved through good-faith negotiation. You may contact us at support@amyloid-test.com to initiate this process. If negotiation fails, disputes shall be resolved through binding arbitration administered under the rules of the Centro de Arbitraje de México (CAM) in Mexico City, or as otherwise agreed. The arbitration shall be conducted in English or Spanish. **Class Action Waiver:** You agree that any dispute resolution proceedings will be conducted only on an individual basis and not in a class, consolidated, or representative action.

We reserve the right to modify these Terms at any time. When we do, we will: - Post the updated Terms on this page with a revised "Last Updated" date - Notify existing customers of material changes via email - Provide reasonable notice before changes take effect Your continued use of our service after changes become effective constitutes acceptance of the modified Terms. If you do not agree with the changes, you must discontinue use of the service.

If any provision of these Terms is held to be unenforceable or invalid, such provision will be changed and interpreted to accomplish its objectives to the greatest extent possible under applicable law, and the remaining provisions will continue in full force and effect.

These Terms, together with our Privacy Policy and Disclaimer, constitute the entire agreement between you and AmyloidTest regarding the use of our service, and supersede any prior agreements or understandings.
